Referrals from your Doctor

If you would like to be seen by a physician at Cancer Care Associates, you will need to obtain a referral from either your primary care physician or a specialist. After a referral is made, you will be contacted by a member of our New Patient Department to set up an appointment.

To assist your physician in making the referral, you can print this page and bring it to your next appointment.

We accept most major insurance including HMOs, PPOs, Medicare and Medi-Cal.

To make a Medical Oncology referral to our Practice, please have your primary physician call our Referral Department at (559) 326-1220 or fax the referral to (559) 326-1228.
